Liability Waiver with Jewish Federation of San Diego
Skatedakah Event 2025

MEDICAL
It is the responsibility of the undersigned to ensure that the participant is medically fit to take part in strenuous on-rink or off-rink activities. As stated below, participation in roller skating and/or roller derby activities presents an inherent risk of injury to person or property. The undersigned certifies that the participant has no known conditions that prohibit or limit participation in any skating or physically strenuous activities held by or in association with Jewish Federation of San Diego or Ringer's Roller Rink and Derby United.

Participant takes part at their own risk and is solely responsible for all insurance coverage. Participants certify that they have adequate insurance to cover any injury or damage they may cause or suffer while participating in, or activities associated with on-rink or off-rink activities.

AUTHORIZATION FOR EMERGENCY MEDICAL TREATMENT
In case of medical emergency, participant grants full power to the above listed organizations to 1) arrange for the transportation, whether by ambulance or otherwise, to a proper facility where emergency medical treatment would normally be administered, including but not limited to, an emergency room of a hospital, a doctor’s office, or a medical clinic; and 2) sign releases as may be required in order to obtain any medical or surgical treatment as is required in the judgment of medical authorities at the facility.

EQUIPMENT AND SKATES
All Roller Skating participants are strongly encouraged to wear safety gear including Knee, Elbow and Wrist Pads, and Helmet. Renting this gear is at the participant’s discretion and cost.

Participant is responsible for inspecting their rental roller skates, protective gear and the skating surfaces at the time of their use. Some surfaces may become slippery when wet. Participant accepts responsibility for inspection and will immediately notify a Ringer's staff member of any unsafe conditions.

INSTRUCTION AND EQUIPMENT
The training, opinions, findings, and conclusions offered are that of the instructors/speakers and do not imply medical expertise or acceptance of any liability. The materials and information presented are for informational purposes only & are not legal advice or counsel. Information gathered should be modified to fit your own personal needs, athletic competence, limitations, and state requirements. All skaters should execute proper safety precautions prior to attempting any skills demonstrated or shown. All participants skate solely at their own risk. All equipment rentals should be inspected by the participant. Participants use loaner/rental skates and safety equipment at their own risk.

MEDIA
Participants may be photographed or videotaped during this event. The undersigned grants Jewish Federation of San Diego the right to use their likeness in marketing or social media, whether now known or hereafter devised, throughout the universe in perpetuity without compensation, reservation or limitation.

CONDUCT
Any patrons on the premises, spectators and participants must behave in a respectful manner to both person and property. Abusive or derogatory language will not be tolerated and will lead to immediate removal from the facility. Behavior which could potentially lead to intentional or unintentional bodily injury or injury to property will lead to immediate removal from the facility. No refunds will be given.

INDEMNIFICATION AND RISK ACKNOWLEDGMENT
In consideration of being allowed to participate in any way in roller skating for the Skatedakah event with Jewish Federation of San Diego, including any such claims which allege negligent acts or omission of Roller Derby 360 LLC, the undersigned acknowledges, appreciates and agrees that:

1. The risk of injuries from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury remains; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and I assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY VOLUNTARILY RELEASE, FOREVER DISCHARGE, AGREE TO INDEMNIFY AND HOLD HARMLESS JEWISH FEDERATION OF SAN DIEGO COUNTY , and/or employees, and other participating skaters, (“RELEASEES”) WITH RESPECT TO ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, INJURY, DISABILITY, DEATH, or loss, or damage to my person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

5. I, for myself and on behalf of my heirs, give permission to the league to authorize emergency medical treatment as it may be deemed necessary by Company for Participant.

I HAVE READ THIS ASSUMPTION OF RISK AGREEMENT AND I ACCEPT RESPONSIBILITY; I FULLY UNDERSTAND ITS TERMS, AND I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. MY SIGNATURE ON THIS DOCUMENT IS BINDING FOR THE DURATION OF MY MEMBERSHIP AND RENEWALS.
